Primary Objective:

To coordinate and provide an opportunity for clients to reach restorative goals after a hospital stay by improving physical, psychosocial and or cognitive functioning. This program aims to reduce the risk of hospital readmission and premature entry into residential aged care by utilising a multi-disciplinary team approach. This vacancy may be available in either Katherine or Darwin subject to negotiation with the successful applicant.
