Primary Objective:

Support the team to achieve the best outcomes for clients by coordinating assessment services and resources in accordance with the Australian Government Aged Care Legislation and Departmental Polices, including conducting clinical assessments and case coordination for clients and families eligible for the Aged Care Assessment Program.
